Vaccines are available with our nurses year-round on Tuesday and Thursday evenings from 5:30pm through 7:30pm. Call the office to make your vaccine appointments during the available scheduled times only. These appointments do not take much time at all so you can be in, out, and on with your busy schedule! **Please note that PPD placements/reads, allergy shots, and special vaccines (hormone, antiobiotic, etc.) cannot be made with the vaccine clinic nurses.**

Measles is common in Europe, Asia, the Pacific, and Africa. Before you travel tell your doctor where you are traveling! Babies as young as 6 months of age can be vaccinated prior to travel. After you travel call your doctor if anyone gets a fever and rash within 3 weeks of returning from your trip. Describe where you traveled. FORMS All physical and/or medication forms must be either dropped off in person at our office, faxed 856-988-9850, or emailed to atrium@advocaredoctors.com. Forms take at least 5 business days to complete and there is a fee of $10 for all physical forms for sports/school registration. There is no charge for medication forms. You can pay via phone with a credit card and we can fax/email forms. Please specify office location that you plan to pick up the form(s); Cedarbrook or Marlton. Please check office hours. Arrangements may be made on a case by case basis to pick up forms after hours if you are unable to fax or email. Thank you for your understanding in this matter!FORMS

MEDICAL RECORDS ALL COMPLETE MEDICAL RECORDS CAN BE ACCESSED VIA THE PATIENT PORTAL LINK ON OUR WEBSITE. PLEASE CALL THE OFFICE AHEAD OF TIME IF YOU ARE MOVING OUT OF STATE TO PREPARE LARGER CHARTS FOR PICK-UP. THERE IS A $20 FEE FOR MEDICAL RECORDS TO BE PAID WHEN PICKING UP IN THE OFFICE. ADVOCARE OFFICE-ADVOCARE OFFICE TRANSFERS CAN HAVE MEDICAL RECORDS SENT ELECTRONICALLY NO CHARGE. MEDICAL RECORDS CAN BE FAXED TO OTHER CARE CENTERS AS WELL WITH PAYMENT COLLECTED OVER THE PHONE PRIOR TO SUBMISSION. THANK YOU!MEDICAL RECORDS
